Video Photograph Submission Guidelines

Feeling Their Presence/Remembering Their Spirit is an opportunity to honor, celebrate and remember those whose have died from cancer. The U-M Cancer Center is collecting photographs of those who have lost their cancer battle for this remembrance video to be displayed during the September 30th Candle Lighting for Hope and Remembrance.

Electronic photographs (if you wish to send photos via US mail, click here):

• Electronic photographs must be in JPEG format (640 x 480 pixels, at least 72 dpi) and saved at the "highest" setting your scanner/photo program allows.
  
  
• Electronic photographs must be received by September 15 and must include a completed online submission form. Open and save online submission form / form EE - Note: this is a rich text format (.rtf) document, so should open for both Mac and PC users

• Send electronic photographs along with completed form EE to: mtowas@med.umich.edu

US Mail Photographs

• Photographs should be no smaller than 2" x 3" (wallet size) and mailed in packaging that prevents damage.
  
  
• Photographs must be received by September 15 and must include a completed submission form (Form MM). Print form MM -- Note: this is a .pdf document
  
  
• Send photographs along with completed form MM to:

U-M Cancer Center will attempt to include as many photographs as possible during the program but cannot guarantee that all photographs submitted will be used.

PLEASE NOTE - Photographs may NOT be sent by fax. We cannot accept slides.
