Help at Home

Coordinating the specific services and products patients need to maintain their treatment and recovery regimens at home is the purpose of the U-M Health System's Discharge Planning Service. Working with individual patients and the Cancer Center's social work staff, discharge planners can coordinate the delivery of services and products needed directly to the home. Examples of the services they coordinate include:

  • Medical equipment
  • Infusion Therapy
  • Dressing and IV catheter care
  • Home nutritional services
  • Hospice Care
  • Nursing home placement
  • Rehabilitation care
  • Ambulance transportation
  • Community support services
