We would like participants to keep us informed of any changes in their cancer health status (e.g., you have developed cancer, your prostate cancer has recurred, you have a more recent PSA value to report, a new family member has been diagnosed with cancer) on a continuing basis. Access to the survey has been designed to ensure complete confidentiality (the survey instrument is maintained on a restricted University of Michigan server and only members of our research team are able to see the responses). So that only participants will use the survey, we have added a password at the start of the survey: *****. We encourage you to use this new convenient means of keeping us up-to-date about any changes in your cancer health status. For those of you who may not have access to the Internet, would prefer to report health updates via phone or a paper survey, or do not wish to participate in the survey, please contact us at the following address so that you can request a mailed copy or let us know that you would prefer to not complete the survey.
