New PCGP On-Line Health Update Survey

The health update information we received from many of you as a result of our Summer 2004 Ethnicity Questionnaire was so helpful that we have added a "health update" link to our webpage. We would like participants to keep us informed of any changes in their cancer health status (e.g., you have developed cancer, your prostate cancer has recurred, you have a more recent PSA value to report, a new family member has been diagnosed with cancer) on a continuing basis. Access to the survey has been designed to ensure complete confidentiality (the survey instrument is maintained on a restricted University of Michigan server and only members of our research team are able to see the responses).

So that only participants will use the survey, we have added a password at the start of the survey: *****. We encourage you to use this new convenient means of keeping us up-to-date about any changes in your cancer health status.

For those of you who may not have access to the Internet, would prefer to report health updates via phone or a paper survey, or do not wish to participate in the survey, please contact us at the following address so that you can request a mailed copy or let us know that you would prefer to not complete the survey.

Please also use the postcard to report any address/phone number/email updates.
