Candle Lighting for Hope and Remembrance - Photo Submission:
Information and Instructions

Submit a photo for the Remembrance Video by Friday, August 29, 2008

Please be advised of the following submission guidelines:

• Photographs should be no smaller than 2" x 3" (wallet size) and mailed in packaging that prevents damage.
• Photographs must be received by August 29, 2008 and must include a completed submission form - opens as a .pdf document
• Photographs from previous years are not automatically repeated in following years. Photographs must be resubmitted yearly.
• Photographs submitted without a completed form will not be displayed.
• Photographs may NOT be sent by fax.
• We cannot accept slides.
• Photographs can be sent US Mail to:

• Unfortunately, photographs cannot be returned.

U-M Cancer Center will attempt to include as many photographs as possible, but cannot guarantee that all photographs submitted will be used. All photographs must be identified and a photo release signed to be displayed.
